Lunalilo Home offers Expanded Care Services for residents who require a higher level of assistance in their Activities of Daily Living and their Instrumental Activities of Daily Living . Expanded Care is provided on a space-available basis on the first floor of the Home. Hawaiʻi state regulations require that the resident have an independent case manager to oversee the Expanded services provided by the Home.

The resident, their family and/or responsible party are accountable for the cost of the independent case manager. It is the policy of Lunalilo Home to primarily admit Hawaiians who are poor, destitute and infirm. Such individuals must satisfactorily demonstrate Hawaiian extraction, financial situation, and health status in keeping with the facilities and services provided by the Home. To the extent there are available bed spaces at the Home and as provided by law, preference will first be given to elderly Hawaiians and then to the general elderly community who are able to pay the full cost of residence at the Home.
The occupational therapists focus on rehabilitation and recovery of functional independence, which includes skills for eating, bathing, toileting, cooking, dressing, and grooming.
